Estetica Istanbul Medical Team
If you're considering a hair transplant, you've probably come across two acronyms everywhere: FUE and DHI. Both are modern, minimally invasive techniques that deliver natural-looking results — but they work differently, cost differently, and suit different situations.
At Estetica Istanbul, we perform both FUE and DHI procedures daily. Our surgeons will always recommend the technique that best fits your specific hair loss pattern, goals, and donor area characteristics. In this guide, we break down everything you need to know so you can have an informed conversation with your surgeon.
FUE is the most widely performed hair transplant technique worldwide. It involves extracting individual hair follicles from the donor area (typically the back and sides of the head) using a micro-punch tool, then implanting them into tiny incisions in the recipient area.
How it works:
Step 1 — Extraction: Using a micro-punch (0.6–0.9mm diameter), the surgeon extracts individual follicular units one by one from the donor area. This leaves tiny, virtually invisible dot scars that heal within days.
Step 2 — Channel creation: The surgeon creates micro-incisions (channels) in the recipient area, carefully controlling the angle, direction, and depth to ensure natural-looking growth.
Step 3 — Implantation: Extracted follicles are placed into the prepared channels using micro-forceps.
Best suited for: Patients needing large graft counts (3,000–5,000+ grafts), those who want to wear their hair very short (minimal scarring), and most standard hair loss patterns (Norwood 2–6).
DHI is an advanced variation of FUE that uses a specialized implantation pen called the Choi pen. The key difference: DHI combines the channel creation and implantation steps into one, meaning follicles are implanted directly without pre-made incisions.
How it works:
Step 1 — Extraction: Same as FUE — individual follicles are extracted with a micro-punch tool.
Step 2 — Direct implantation: Each follicle is loaded into a Choi implanter pen and directly inserted into the scalp at a precise angle and depth. No pre-made channels are needed.
Best suited for: Patients who want maximum density in specific areas (especially the hairline), those who prefer a shorter recovery, and patients who want to minimize shaving of the recipient area.
Technique approach: FUE uses a two-step process (create channels, then implant). DHI combines both into one step with the Choi pen.
Density potential: DHI can achieve slightly higher density in targeted areas because the surgeon has more precise control over follicle placement. For overall coverage of large areas, FUE is equally effective.
Graft count per session: FUE can handle larger sessions (up to 5,000+ grafts). DHI is typically better suited for sessions of up to 3,500 grafts due to the more time-intensive implantation process.
Scarring: Both techniques leave minimal, virtually invisible scarring. FUE leaves tiny dot scars in the donor area. DHI has the same donor scarring plus slightly less trauma in the recipient area.
Recovery time: FUE recovery takes approximately 7–10 days for the initial healing phase. DHI recovery is slightly faster — typically 5–7 days — because there are no pre-made incisions in the recipient area.
Shaving requirement: FUE typically requires shaving the entire donor area. DHI can sometimes be performed with partial or no shaving in the recipient area ("unshaven DHI"), which is popular with patients who have longer hair.
Cost: At Estetica Istanbul, FUE starts from €1,990 while DHI ranges from €2,500–€3,500, reflecting the additional precision and time required.
Procedure time: FUE takes 4–8 hours depending on graft count. DHI takes 5–10 hours for a comparable number of grafts due to the more meticulous implantation process.
Both FUE and DHI follow the same general results timeline:
Weeks 1–2: Initial healing. Tiny scabs form and fall off. Some patients experience mild swelling for 2–3 days.
Weeks 2–4: "Shock loss" phase — transplanted hairs fall out. This is completely normal and expected. The follicles remain alive beneath the surface.
Months 3–4: New growth begins. Fine, thin hairs start to emerge from the transplanted follicles.
Months 6–8: Significant growth becomes visible. Hair thickens and gains texture. Most patients see 60–70% of their final result at this stage.
Months 10–12: Full results. Transplanted hair has reached its final thickness and length. The result is permanent — transplanted follicles are genetically resistant to the hormone (DHT) that causes pattern baldness.
Note: Individual results may vary. Graft survival rates depend on patient health, post-operative care, and individual factors. Typical graft survival rates range from 85–95% at experienced clinics.
Choose FUE if: You need a large number of grafts (3,000+), you want to keep costs lower, you're comfortable shaving the donor area, and you have a standard hair loss pattern that requires broad coverage.
Choose DHI if: You want maximum density in the hairline or front area, you prefer a slightly faster recovery, you want to avoid shaving the recipient area, or you're willing to invest more for the added precision.
Our honest recommendation: For most patients, FUE delivers outstanding results at a better price point. DHI is the premium choice when hairline precision and density are the top priorities. During your consultation, our surgeon will examine your specific situation and recommend the technique that will give you the best possible result.
