Butt Implants

Buttock implants — silicone implants placed in the gluteal muscle or above it — augment the buttocks with a permanent synthetic volume. It is the right operation for a narrow subset of patients, and the wrong operation for most patients who ask for it. The reason is simple: for the large majority of patients seeking gluteal augmentation, BBL (fat-grafting) produces a more natural result using the patient's own tissue, addresses problem areas like the flanks and lower back in the same session, and carries a lower lifetime complication profile.

We say this on the front of the page because honest candidate selection is the first and most important step of this consultation. If BBL is the right operation for you, we will say so and will not sell you implants. If implants are genuinely the right operation, we explain why and proceed with careful technique.

Who butt implants are suitable for

The right candidate falls into one of a small number of groups. Lean patients (BMI under 21–22) with insufficient donor fat for a meaningful BBL — without enough fat to transfer, BBL cannot produce the projection the patient wants, and implants become the remaining surgical option. Patients who want more projection than BBL can deliver — the projection of a BBL is limited by tissue-graft survival and the contour of the existing buttock; implants can deliver more projection if that is the specific goal. Patients who have already had BBL and want additional volume — implants can be placed on top of a prior fat-grafting result when additional projection is desired. Patients with significant fat-loss after weight loss but who want a fuller buttock appearance — a specific post-bariatric subset.

Who should not have this operation: patients with adequate donor fat for a BBL (BBL is the better choice); patients with any active skin condition in the gluteal region; patients with unrealistic projection expectations (there are anatomical limits — exceeding them produces complications); patients who cannot tolerate a six-week recovery with strict avoidance of sitting directly on the buttocks.

Butt implants vs BBL — the honest decision tree

For most patients, BBL is the better operation. It uses your own tissue (no foreign body, no lifetime implant maintenance), addresses the waist and flanks in the same session (a significant aesthetic benefit for the "hourglass" contour that most patients actually want), and has a lower infection and capsular contracture profile long-term.

Implants are the better choice only in the narrow indications above. At consultation we measure your donor-fat reserves, discuss your projection goal, and make an honest recommendation. A clinic that performs implants on every patient asking for "a bigger butt" is performing the wrong operation on most of them.

We also honestly discuss the third option — no surgery. Hip dips, subtle shape concerns, and volumes that fall within a normal anatomical range may be better served by glute training and targeted exercise over twelve to eighteen months. We tell patients when this is the right answer.

Implants — placement options

Two main placement planes are used. Each has distinct trade-offs.

Intramuscular placement: the implant is placed inside the gluteus maximus muscle. Produces a more natural feel, better long-term stability, lower visibility of the implant edges. The technically more demanding approach but the one we prefer in most cases. Higher volumes of tissue above the implant reduce the risk of implant edges being palpable.

Subfascial placement: the implant is placed above the muscle, under the deep fascia. Technically simpler and recovery slightly faster, but higher risk of implant edges being visible or palpable, especially in lean patients. Used selectively.

The choice is technique-dependent and discussed at consultation.

The surgery, step by step

Performed under general anaesthesia in a JCI-accredited hospital, with a dedicated anaesthesiologist present. Duration two to three hours. One or two overnight stays in hospital are standard — longer than breast augmentation because the post-operative positioning is more restrictive.

The incision is placed in the intergluteal cleft (between the buttocks), hidden, and closed with fine sutures. The implant is inserted through this single midline incision and positioned in the chosen plane. Closure is meticulous — the incision sits in an anatomical region particularly prone to wound breakdown if closed poorly.

Recovery

Recovery for butt implants is longer and more demanding than BBL. The patient does not sit directly on the buttocks for six weeks — the same "no sitting" rule as BBL — and physical activity is more restricted in the first month because the implant capsule must form without disturbance.

Weeks 1–2

No sitting on the buttocks — use a BBL cushion placed under the thighs to offload the implant zone. Sleeping on the stomach or side (not the back). Moderate pain and swelling, controlled with standard analgesics and muscle relaxants. Antibiotic prophylaxis for ten days. No driving. Shower after day three, with wound care instructions.

Weeks 3–6

Gradual return to daily activity, still avoiding direct sitting on the buttocks. Swelling resolves progressively. Desk work with BBL cushion from week three. Light walking permitted.

Months 2–6

Progressive return to normal activity. Direct sitting resumes around week six or seven. Gym activity from month two, lower-body resistance training cautious until month three. Final shape settles between three and six months.

Risks and realistic expectations

Honest risks — and they are higher than for breast implants because the buttocks bear weight daily: capsular contracture (the implant capsule can tighten over years, producing a firmer buttock and requiring revision); implant malposition or rotation (especially if positioning instructions are not followed); infection (higher rate than other body implants because of the anatomical region); wound dehiscence at the intergluteal incision (why meticulous closure matters); seroma; temporary altered sensation; long-term risk of implant rupture or shell failure requiring exchange.

Implants are not lifetime devices. Most butt implants will require replacement or removal at some point over decades. We explain this explicitly at consultation — implants are a commitment to potential future surgeries, not a one-time intervention.
