A Brazilian Butt Lift — BBL — uses your own fat to reshape and add volume to your buttocks. Fat is harvested from the areas where you have excess (abdomen, flanks, thighs, back) and re-injected into the buttocks along carefully-defined anatomical planes. The double benefit is what makes it one of the most-requested procedures in cosmetic surgery: you gain curves where you want them and lose volume where you don't.
The BBL is also the most scrutinised procedure in plastic surgery. Between 2013 and 2019 it carried the highest mortality rate of any cosmetic operation ever recorded, driven almost entirely by intramuscular fat injection causing fatal fat embolism. The international response was decisive: subcutaneous-only injection, ultrasound guidance, surgeons with high annual volume. Every BBL we perform follows those standards — and because they are non-negotiable, we start the conversation there.
You are a suitable BBL candidate if you have enough donor fat — roughly four to six litres between waist, flanks and back — you are in good general health, you are a non-smoker or willing to stop, and you have realistic expectations about what a single session can achieve.
If you are very lean, we may suggest reaching a slightly higher target weight before surgery, or discuss gluteal implants as an alternative. If you smoke, you must stop at least four weeks before surgery — nicotine drastically reduces transplanted fat survival. If you are expecting a transformation into a silhouette your anatomy cannot support, we will tell you in consultation, and if the outcome you want is not safe we won't operate.
BBL is performed under general anaesthesia in a JCI-accredited hospital, with a dedicated anaesthesiologist present throughout. Surgery takes three to four hours depending on the volume of fat harvested and the complexity of body contouring.
The surgeon harvests fat from the agreed areas — most commonly abdomen, flanks (love handles), lower back and inner thighs. This is the contouring half of the operation: the donor areas are sculpted as they are emptied, and for many patients the improvement here is as visible as the buttock change.
Harvested fat is centrifuged to separate viable adipocytes from blood, anaesthetic fluid and damaged tissue. Only clean, living fat cells are re-injected. This is the step that determines how much of the transfer actually survives.
Fat is injected in micro-droplets into the subcutaneous layer of the buttocks — **never into muscle**. This is the founding safety rule of modern BBL. Where the intramuscular injection of the 2010s caused fatal fat embolisms by breaching large gluteal vessels, subcutaneous-only injection removes that mechanism entirely. Ultrasound guidance confirms the cannula stays above muscle throughout. Our surgeons follow the Multisociety Gluteal Fat Grafting Task Force guidelines without exception, and that discipline is the single biggest reason our safety record is what it is.
Entry points are closed. A compression garment is applied over liposuction areas only — never directly onto the buttocks, because compressing freshly-grafted fat would kill the transferred cells.
Some patients notice an unusual smell in the weeks after surgery, usually coming from the liposuction areas. It is a by-product of normal healing — reabsorption of lymphatic fluid, metabolism of residual fat cells, inflammatory response. It is not a warning sign. It typically resolves within three to four weeks as wounds close and swelling subsides.
Gentle wound hygiene following the surgeon's protocol keeps it under control. Contact us immediately if the smell is accompanied by fever, localised redness, increasing pain, or unusual discharge — those are signs of possible infection that need prompt treatment.
Sleep on your front or your side. No direct sitting on the buttocks — use a BBL cushion under the thighs to keep pressure off the graft. Swelling peaks in the first few days then improves steadily. Most patients manage pain with basic analgesics.
Manual lymphatic drainage starts, which improves swelling and final shape. Gentle walking is encouraged from day one; desk work is manageable from the end of week one. Sport stays off the list.
Full recovery. You can sit normally, return to light exercise, and see the shape stabilising. Most of the swelling has resolved.
Final result. The fat that survives the transfer — typically sixty to seventy per cent of the volume injected — is now permanent. It behaves like any other body fat: it grows if you gain weight and shrinks if you lose.
Not all transferred fat survives. Surgeons compensate with slight over-grafting — injecting more than the final volume you want, knowing some of it will be reabsorbed in the first three months. After month three, what remains is permanent.
Maintaining a stable body weight is the single most important factor for preserving the result. Significant weight loss after BBL reduces volume; significant weight gain distorts the shape, because transplanted fat grows just like native fat.
BBL had a reputational crisis in the 2010s because of fat embolism deaths caused by intramuscular injection. British, American, Australian and European plastic surgery societies all responded, and some — notably BAAPS in the UK — went as far as recommending members stop performing BBL altogether until standards tightened. That tightening has happened. The current standard is subcutaneous-only injection, ultrasound guidance where indicated, and high-volume surgeons. Our team operates exclusively under those standards, and that is the technical reason our complications record is what it is.
Standard surgical risks apply — infection, haematoma, asymmetry, seroma — and are managed with close post-operative follow-up. Rare but serious risks are discussed in full at consultation, not minimised to close a sale.
BBL is often combined with tummy tuck (the "mommy makeover" when breast is added), lipo 360, or additional liposuction. Combining procedures in a single operative session saves recovery time and travel cost compared to separate surgeries. Your surgeon will tell you honestly whether combining is safe in one session or whether your case is better staged.

A clinic advertising BBL at €1,800 is cutting costs in exactly the places you shouldn't: reduced anaesthesiologist time, no ultrasound guidance, less-experienced surgeons who may default to intramuscular injection to "add more volume", no serious follow-up on fat reabsorption. The BBL stories that reached the news — the Leah Cambridge case, the Miami cluster, the Dominican Republic fatalities — did not come from JCI-accredited operating theatres. They came from the places where the corners that matter most had been cut.
