A Brazilian Butt Lift — BBL — uses your own fat to reshape and augment the buttocks. Fat is harvested from areas where you want less of it (abdomen, flanks, thighs, back) and re-injected into the buttocks in precise anatomical layers. The double benefit is what makes the procedure so popular: you get curves where you want them, and a slimmer silhouette where you don't.
BBL is the most common buttock augmentation procedure worldwide because it uses living tissue — no implants, no foreign body, and a result that looks and feels like your own anatomy. It is also a technically demanding operation. The difference between a good BBL and a dangerous one is surgeon experience, volume, and strict adherence to safe injection planes.
The ideal BBL candidate has enough harvestable fat — roughly four to six litres across the waist, flanks, and back — is in good general health, does not smoke, and is realistic about what one session can achieve.
BBL is performed under general anaesthesia in a JCI-accredited hospital, with an experienced anaesthesiologist present throughout. The operation takes three to four hours depending on the amount of fat harvested and the complexity of the body contouring.
The surgeon harvests fat from the agreed donor sites — most commonly the abdomen, flanks (love handles), lower back, and inner thighs. This is the contouring half of the operation: the areas we take fat from are also sculpted.
The harvested fat is centrifuged to separate pure fat cells from blood, anaesthetic fluid, and damaged tissue. Only clean, viable adipocytes are injected. This step is where graft survival is won or lost.
Fat is injected in micro-droplets into the subcutaneous layer of the buttocks — never into muscle. This is the critical safety rule: subcutaneous-only injection avoids the fat embolism risk that made older intramuscular BBL techniques dangerous. Our team follows the international multi-society safety guidelines without exception.
Tiny entry points are closed. A compression garment is applied to the liposuction areas only — never directly on the buttocks, because compressing freshly grafted fat would kill the cells you just transferred.
Some patients notice a distinctive odour in the weeks after BBL, usually from the liposuction areas. It is a byproduct of the normal healing process — reabsorption of lymphatic fluid, metabolism of fat cells, and the body's inflammatory response. It is not a warning sign. It typically resolves within three to four weeks as wounds heal and swelling reduces.
Gentle wound hygiene, following your surgeon's cleaning protocol, keeps it in check. Contact us immediately if the odour is accompanied by fever, localised redness, increasing pain, or abnormal discharge — those signal a possible infection that needs prompt treatment.
Sleep on your stomach or side. No sitting directly on the buttocks — use a BBL pillow under the thighs so pressure stays off the graft. Swelling peaks in the first few days, then steadily improves. Most patients manage pain with basic painkillers.
Lymphatic massage starts, improving swelling and shape. Light walking is encouraged from day one; office work is fine by the end of week one. Exercise stays off the list.
Full recovery. You can sit normally, resume light exercise, and see the shape settling. Swelling is nearly gone.
Final result. The fat that survives the graft — typically sixty to seventy per cent of what was injected — is now permanent. Your new shape will change only with normal weight fluctuation from here.
Not all transferred fat survives. Surgeons compensate by over-grafting — injecting more than the final target volume, knowing some will resorb in the first three months. Beyond the three-month mark, what is left is permanent. It behaves like any other fat: it grows with weight gain and shrinks with loss.
Maintaining stable body weight is the single most important factor in preserving your result. Significant weight loss after BBL will reduce volume; significant weight gain will distort the shape.
BBL had a reputation problem in the 2010s due to fat embolism cases linked to intramuscular injection. International plastic surgery societies responded with strict guidelines: subcutaneous-only injection, ultrasound guidance where appropriate, and high-volume operator experience. Our surgeons operate exclusively within these guidelines.
Standard surgical risks apply — infection, haematoma, asymmetry, seroma — and are managed by attentive post-operative care. Rare but serious risks are discussed fully during consultation, not glossed over.
BBL is frequently combined with a tummy tuck (the "mommy makeover" when paired with breast work), lipo 360, or liposuction of additional areas. Combining in one operating session saves recovery time and travel cost compared to staging. Your surgeon will tell you honestly if a combination is safe in one session or should be split.
